How IRMAA works in 2026

Everyone with Part B pays the standard premium of $202.90 a month in 2026 (up from $185.00 in 2025). If your modified adjusted gross income on your 2024 tax return was above the first threshold, you also pay an Income-Related Monthly Adjustment Amount on Part B and, if you have drug coverage, on Part D. CMS published the 2026 amounts on November 14, 2025.

What income does IRMAA use?

Modified adjusted gross income (MAGI): your adjusted gross income plus tax-exempt interest. For 2026 premiums, Social Security uses your 2024 federal tax return.

Is IRMAA a cliff or a gradual increase?

A cliff. Earning one dollar over a threshold moves you into the next tier for the entire year. Managing income near a threshold, for example with Roth conversions or charitable distributions, can matter.

How is IRMAA collected?

The Part B surcharge is added to your Part B premium and, for most people, deducted from the monthly Social Security payment. The Part D surcharge is billed separately from your drug plan premium, also usually through Social Security.

Can I get IRMAA reduced?

If you had a life-changing event such as retirement, reduced work hours, divorce, or loss of a spouse, you can file Form SSA-44 and ask Social Security to use a more recent year's income.

Does IRMAA apply to Medicare Advantage?

Yes. IRMAA is charged on Part B and Part D regardless of whether you have Original Medicare or a Medicare Advantage plan.
